Thank you for your interest in evaluation and treatment in my clinic. Please become familiar with my policies and procedures. They often shape one's decision on moving forward to become a client. I operate the clinic by myself. Please be patient when waiting for a response to correspondence. 

Financial Policy

FINANCIAL POLICY AND FEE SCHEDULE

I do not accept or file with any insurance companies. This section explains how this policy effects your experience with me.

IN A NUTSHELL

  • Payment is due at time of service

  • Cash or Card are the only forms of payment accepted

  • Credit, debit and most HSA cards are accepted

  • We don't participate in motor vehicle accident or workman's comp cases

  • We can keep your card on file 

  • We can provide you a super-bill so you can file with your insurance on a quarterly basis. Please let us know in advance if you would like this service.​

 

Dr. MURER'S FEE SCHEDULE​​​

  • 60 Minute New Patient Visit: $170.00

  • 30 Minute Follow Up Visit:      $85.00

  • On site professional athlete treatment: Call for details

 

 

 

FOR PATIENTS WITH MEDICARE

​

Medicare has a unique set of rules governing chiropractors that cause your experience with me to be governed

differently than it would be at your medical doctor or physical therapist.

 

Brian Murer, DC is a NON-PARTICIPATING provider in Medicare.

​

WHAT DOES THIS MEAN FOR YOU AS A MEDICARE PATIENT IN MY CLINIC? 

​

  • Per Medicare rules, you will be required to sign an Advance Beneficiary Notice (ABN) 

  • We will only accept patients that elect to choose D on their ABN. You are free to elect other options, but unfortunately we will choose not to accept your case.

  • By electing to choose D on the ABN: 

    • You will not be able to file to Medicare yourself​

    • You will not be able to file to a Medicare supplement

    • You are acknowledging your status as a fee for time of service patient

            Thank you for your understanding in this matter.

POLICY ON MINOR CHILDREN

It is the policy that minor children (under 18 years of age), need to be accompanied by an adult over 18. 

​

There will be no special circumstances that overrides this policy.

Minor children who show to our office without an adult over 18 will be turned away and may be charged for the amount of the appointment.

CANCELLATION AND NO SHOW/NO CALL POLICY

 

I require 24 hours notice for cancellations. 

​

I understand that, sometimes life events may cause you to have to cancel your appointment without 24 hours notice or just miss your appointment all together,

but we reserve the right to assess a fee proportional to the cost of your appointment for those who habitually cancel or no show/no call. 

​

NEW PATIENT APPOINTMENTS: If a new patient appointment is cancelled with less than 24 hours notice,

the cost of that missed appointment must be paid before you will be allowed to reschedule.
